Dental Plans - Union of Pitt Faculty

All information presented on this page, including links to plan documents and descriptions, is specific to the plan year of July 1, 2025 - June 30, 2026.

Eligible Pitt employees may choose among three dental plans offered by United Concordia Dental, including a dental health maintenance organization (DHMO) plan and the traditional indemnity program with two “flex” plan designs to meet specific dental care needs.

The Pennsylvania DHMO, Concordia Plus Managed Care (PA Only), includes 100% coverage of preventative services every six months from date of service, as well as access to University Dental Health Services. The traditional indemnity programs, Concordia Flex I and II, offer access to a broad network of dentists, as well as full coverage preventive services when performed by an in-network dentist.

University Faculty and Staff employees who choose to receive dental care through the University of Pittsburgh’s School of Dental Medicine will receive a 10% discount on all services not covered by insurance. Please note that this discount is subject to change at the discretion of the University of Pittsburgh’s School of Dental Medicine. Concordia Plus Managed Care (PA Only) DHMO

Administered by United Concordia, the Concordia Plus Managed Care (PA Only) Dental Heath Maintenance Organization (DHMO) plan offers a limited network of providers within the DHMO Concordia Plus General Dentist network within the Western PA area. The United Concordia Managed Care (PA Only) DHMO plan requires the selection of a Primary Dental Office (PDO). If a PDO is not designated, University Dental Health Services will be designated. Each participant can have their own PDO designation. All dental services, including emergency dental services must be coordinated through the designated PDO. Otherwise, services will not be covered. There is no out-of-network coverage.

To change a PDO designation:

  • For all existing participants: in order to change the designated PDO, employees must contact Untied Concordia directly at 1-877-215-3616 prior to seeing services.
  • Any changes to the designated PDO made after the 28 of the month will be effective after two (2) months.
  • If the currently designated PDO leaves the DHMO network, it would require a change in PDOs within the DHMO network.

When you use dental providers who are part of the DHMO network:

  • There is no annual deductible or annual plan-year maximum.
  • Preventative care services, such as cleanings and exams, are covered at 100% every six months from date of service.
  • Provides partial coverage of orthodontics for eligible dependents only up to age 19.

Important Considerations for Selecting Concordia Plus Managed Care (PA Only) DHMO

If your Home Address on file with the University is outside of Western Pennsylvania, you may want to carefully evaluate the suitability of the Concordia Plus Managed Care (PA Only) DHMO Plan as it has regional coverage limitations that could affect access to care. The in-network service area for this plan is restricted to Western Pennsylvania with limited provider access outside of this region.

Concordia Flex I

Administered by United Concordia, the Concordia Flex I Preferred Provider Organization (PPO) plan offers a broad network of providers within the Advantage Plus network. The Concordia Flex I plan does not require the selection of a Primary Dental Office (PDO).

When you use dental providers who are part of the Advantage Plus network:

  • Preventative care services, such as cleanings and exams, are covered at 100% every six months from date of service after the deductible is met when services are performed by an in-network dentist. When services are performed by an out-of-network dentist, balance billing may occur up to the dentist charge.

  • No annual deductible for preventative care services (such as cleanings and exams), diagnostic, and orthodontic services for eligible dependents only up to age 19 when services are performed by an in-network dentist.
  • Preventative care services, such as cleanings and exams, are covered at 100% every six months from date of service when services are performed by an in-network dentist. When services are performed by an out-of-network dentist, balance billing may occur up to the dentist charge.
  • Provides partial coverage of orthodontics for eligible dependents only up to age 19.

Dental Benefit Providers

To locate participating providers in United Concordia's network:

  1. Go to unitedconcordia.com and select "Find a Dentist."
  2. Elect to search by location or provider name and complete search fields.
  3. Select your network:
  • Pennsylvania DHMO Plan Primary Dental Office participants select DHMO Concordia Plus General Dentist in the “Select Network” search box located in Western PA ONLY.
  • United Concordia FLEX I or FLEX II participants select Advantage Plus.
  1. Review search results:
  • Practices that come up in this section are covered within the network you are searching.
  • For the Pennsylvania DHMO Concordia Plus General Dentist networks, select the desired practice to find the Provider ID to use for the PDO Code.* Use the 9 digit Provider ID, including leading zeros, to designate the PDO(s) when enrolling.
  • If you are searching by a provider's name, use their last name only in the search bar.

*Note: If electing the United Concordia PLUS Managed Care DHMO (PA Only) plan, you are required to select a network primary dental office (PDO) in Western PA ONLY. Please use the Provider to designate the PDO when enrolling. Claims will be denied for services provided by anyone other than the designated PDO. If a PDO is not elected at enrollment, University Dental Health Services will be assigned. Each participant can have their own PDO. To update PDO after initial enrollment to a different Western PA PDO, call United Concordia at 1-877-215-3616.

Obtain a Dental ID Card

If you need to access your dental ID card, you can do so easily through United Concordia’s online portal. Follow these steps:

  1. Visit the United Concordia website – Go to UnitedConcordia.com and click on "MyDentalBenefits."
  2. Log in or create an account – If you haven’t registered yet, you’ll need to set up a personal MyDentalBenefits account.
  3. Access your ID card – Once logged in, you can view, download, or print your digital member ID card directly from the homepage.
  4. Use the mobile app – You can also access your ID card through the United Concordia Dental app, available on the Apple App Store and Google Play.

If you need a physical copy mailed to you, you can request one through your online account or contact United Concordia’s customer service at 1-877-215-3616.

Note: Benefits premiums for health and welfare coverage are deducted from the employee’s paycheck. These deductions can occur on a pre-tax or after-tax basis depending on the election and employee’s job category.
