Accessing the UPMC MyHealth Questionnaire

Faculty and Staff

1. Log in to your UPMC MyHealth Online account (via Pitt Passport)
2. Select the hamburger menu (three lines) on the top left corner
3. Select Better Health and Wellness, then select MyHealth Questionnaire
4. Read and agree to the terms and conditions
5. Select the Begin assessment button to get started

Covered Spouses/Domestic Partners

1. Visit www.upmchealthplan.com/members
2. Log in if you already have an account; if you do not have an account, select Register for MyHealth Online, then accept the terms, enter the requested information and follow the account setup instructions
3. Once logged in, select Better Health and Wellness, then select MyHealth Questionnaire
4. Read and agree to the terms and conditions
5. Select the Begin assessment button to get started
