Personal Health Information

In accordance with the Health Insurance Portability and Accountability Act of 1996, a group health plan such as the University's group health plan must give plan participants notice of their rights and the plan's legal duties with respect to protected health information (PHI) and how the plan may use and disclose PHI. In spring 2003, the plan issued a privacy notice outlining these rights and duties. At this time, we would like to remind you that you have a right to receive a copy of the privacy notice. The privacy notice can also be obtained from the Office of Human Resources Benefits Department at 200B Craig Hall.

The plan also is required to protect PHI in electronic form (EPHI). The University has implemented reasonable and appropriate safeguards to protect the confidentiality, integrity, and availability of PHI and EPHI that it creates, receives, maintains, or transmits on behalf of the plan.
