All information presented on this page, including links to plan documents and descriptions, is specific to the plan year of July 1, 2025 - June 30, 2026.

Eligible Pitt employees may choose from two vision plans offered by Davis Vision by MetLife that provide flexibility in providers and coverage to meet vision care needs.

Important Note: The name of the University’s vision insurance changed from Davis Vision, to Davis Vision by MetLife, effective 7/1/23. While the coverage itself did not change, the name of the insurance company changed. When seeking vision services through a provider, please ensure that you are providing the full name of “Davis Vision by MetLife” on any forms and/or to the provider’s office upon checking in for your appointment.

Administered by Davis Vision by MetLife, the Fashion Excellence Preferred Provider Organization (PPO) plan offers a broad network of providers within the Davis Vision by MetLife Network.

When you use vision care providers who are in-network:

  • Comprehensive eye examination covered at 100% once every 12 months from last date of service.
  • One pair of eyeglass lenses covered at 100% with an allowance towards frames once every 12 months from last date of service OR an allowance towards contact lenses (including evaluation and fitting) once every 12 months from the last date of service.

Administered by Davis Vision by MetLife, the Designer Gold Preferred Provider Organization (PPO) plan offers a broad network of providers within the Davis Vision by MetLife Network. This plan is a “buy-up” option from the base Fashion Excellence Plan.

When you use vision care providers who are in-network:

  • Comprehensive eye examination covered at 100% once every 12 months from last date of service.
  • One pair of eyeglass lenses covered at 100% with an allowance towards frames once every 12 months from last date of service OR an allowance towards contact lenses (including evaluation and fitting) once every 12 months from the last date of service.

The allowances for this plan are higher than the allowances in the Fashion Excellence plan.

Benefits eligibility and listings of participating providers and services may be found on the Davis Vision by MetLife website.

To locate participating providers in the Davis Vision by MetLife network:

  1. Go to MetLife.com.
  2. Scroll to “How can we help you” and select “Find a vision provider.”
  3. Select “MetLife Vision - Davis”.
  4. Complete the demographics section (location, mile radius, etc.).
  5. Click “Search Now” to obtain a provider list based on your inputs in step 4.

Please Note: When creating an account with MetLife MyBenefits, please add an appropriate amount of zeroes to the front of your employee number to ensure it is nine digits long (e.g. 000123456).

ID cards are accessible via single-sign-on (SSO) from my.pitt.edu.

  1. Login to my.pitt.edu
  2. Search “MetLife My Benefits”
  3. Select “View ID Cards” 

You may also access vision cards through MetLife’s online portal. Follow these steps:

  1. Visit the MetLife website – Go to MetLife Vision Insurance and log in or register for an account.
  2. Access your ID card – Once logged in, navigate to your benefits section where you can view, download, or print your digital member ID card.
  3. Request a physical card – If you need a physical copy mailed to you, you can request one through your online account or contact MetLife’s customer service at 1-888-777-7418.

Note: Benefits premiums for health and welfare coverage are deducted from the employee’s paycheck. These deductions can occur on a pre-tax or after-tax basis depending on the election and employee’s job category.
