Students: Certification of Domestic Partnerships

A completed and notarized Affidavit of Domestic Partnership must be submitted when enrolling or submitting a life event (qualified status change) through eBenefits, the student health insurance portal. Documentation to verify that the domestic partnership meets the criteria described in the University of Pittsburgh Affidavit of Domestic Partnership will be required. Once the completed affidavit and supporting documentation has been submitted, it will be reviewed and the student will be contacted with the approval, denial, or if additional information is required.
