Dental Plans and Rates

Dental coverage is offered through three benefit plan options, including a very popular dental health maintenance organization (DHMO) plan and a traditional indemnity program with two "flex" plan designs.

The DHMO, Concordia Plus, includes 100 percent coverage of cleanings, exams, and other preventative services, as well as access to University Dental Health Services, a comprehensive dental practice located on the Pittsburgh campus. The traditional indemnity programs, Concordia Flex I and II, offer access to a broad network of dentists, as well as full coverage of preventative services.

Dental Plans and Monthly Premiums

 Concordia Plus DHMO

  • Requires selection of a network primary dental office, claims will be denied for services provided by anyone other than the designated PDO. If a PDO is not elected at enrollment, one will be assigned. Each participant can have their own PDO.
  • Charges no annual deductible or annual plan-year maximum,
  • Covers 100 percent of the cost of cleanings, exams, and other preventative services, and
  • Provides partial coverage of orthodontics for eligible dependents up to age 19.

Concordia Flex I

  • Allows participants to choose dentists from a broad network, and
  • Covers 100 percent of the cost of preventative services.

Concordia Flex II

  • Allows participants to choose dentists from a broad network,
  • Charges no annual deductible for preventative, diagnostic, and orthodontic services,
  • Covers 100 percent of the cost of preventative services, and
  • Provides partial coverage of orthodontics for eligible dependents up to age 19.

Dental Benefit Providers

To locate participating providers in United Concordia's network:

  1. Go to www.ucci.com.
  2. Click on "find a dentist."
  3. For the Concordia PLUS plan, select “Concordia Plus” from the “My Network Is” drop down menu
    • Use the last 6 digits of the Provider ID to designate the PDO(s) when enrolling.
  4. For the Concordia FLEX I and II plans, select “Advantage Plus” from the “My Network Is” drop down menu
