Coverage for Dependents

 

Spouse

A faculty or staff member’s spouse, under a legally valid existing marriage between two persons is eligible to be covered on the insurance policy.

In the event of a divorce, the ex-spouse must be removed from your policy within 60 days of the divorce being finalized as stated by the date on the official divorce decree. A copy of the divorce decree must be submitted with the appropriate status change paperwork to remove the ex-spouse from your policy. Once the paperwork has been received and processed, COBRA notification will be sent to offer a continuation of coverage for the ex-spouse.

Domestic Partner

To enroll a domestic partner, an approved Affidavit of Domestic Partnership with supporting documentation of a consecutive 12-month relationship is required. Approved affidavits allow enrollment in medical, dental, vision, dependent life insurance, and long-term care coverage. In the event of a dissolution of the domestic partnership, the ex-domestic partner must be removed from your policy within 60 days of the event being finalized as stated by the date on the statement of termination of domestic partnership. A copy of the statement must be submitted with the appropriate status change paperwork to remove the ex-domestic partner from your policy. Once the paperwork has been received and processed, COBRA notification will be sent to offer a continuation of coverage for the ex-domestic partner.

Please note, another Affidavit of Domestic Partnership cannot be filed until one (1) year after a Statement of Termination of Domestic Partner Status of the most recent domestic partnership has been filed with the Office of Human Resources.

Children

General Overview

Unmarried biological children, stepchildren and adopted children who are solely dependent upon you for support are eligible to be covered on your insurance policy up until the age of 26.

Newborns

Newborn children are automatically covered under your policy for the first 31 days from the date of birth. To keep your child covered after that 31 day period, you must submit a request for a qualified status change form and enrollment form to the Benefits Department within 60 days from the child’s date of birth. If the required paperwork is not completed timely within the 60 day status change period, the child’s coverage will not continue.

Child legally placed for adoption or Legal Guardianship

A child legally placed for adoption or a child of whom you have legal guardianship and the duty to support is eligible for coverage on your insurance policy with an order of the court.

Dependent Children up to the age of 26

Dependent Children can remain on your health insurance policies up to the age of 26. Please visit the Coverage for Eligible Dependent Children page for more information.

Disabled Dependents

Eligibility will be continued past the age of 19 for unmarried dependent children who, as medically certified by a physician, are incapable of self-support due to mental retardation, physical disability, mental illness or a developmental disability that started before age 19. UPMC Health Plan will require medical documentation.

Documentation Requirements for Dependents

If you are making an election to cover a spouse, domestic partner, or child(ren) to your medical, dental and/ or vision coverage, documentation of the relationship is required.

For a spouse, please present a Marriage Certificate to the Office of Human Resources, Benefits Department.

For a domestic partner, a completed and notarized Affidavit of Domestic Partnership must be submitted to the Office of Human Resources, Benefits Department.  Documentation to verify that the domestic partnership meets the criteria described in the University of Pittsburgh’s Affidavit of Domestic Partnership will be required. 

For dependent child(ren), a Birth Certificate(s) must be presented. Paperwork for adopted or stepchildren is also applicable. 

Dependent coverage will not be added without the required documentation.

If you are making an election to delete a spouse or domestic partner due to the end of a relationship, a copy of a Divorce Decree or Statement of Termination of Domestic Partner Status is required.
