How to Code the Employee Record
for Short Term Disability
For Departmental Use:

Make Sure the Following is Complete:


  1. Complete the First date of Action (first day of the leave) on the top of the Employee Record.

  2. On Line 9 of the ER under the section labeled "Leave Type" fill in the appropriate leave type (STD, FMLA/STD, FMLA, Etc).

  3. On Line 9 of the ER under the section labeled "Total Leave Period" fill in the "first day" of the leave and the "last day" if known.

  4. On line 9 of the ER under the section labeled "Paid Leave Portion" fill in the "first day" of the leave. The "last day" of the leave will be the last day the employee is paid by using their sick time and any other accrued time the employee decides to use (vacation, personal days, comp time).

  5. On line 9 of the ER under the section labeled "Unpaid Leave Portion" fill in the "first day" with the day after the last day of paid leave date. The "last day" will be the day before they return to work. Fill in if known.
